One or both applicants may attend City Hall to obtain the marriage licence. One support person, or translator if required, is also welcome, but we ask you limit the number of people attending the office with you.
Preferred Date and Time for Appointment
Best efforts will be made to accommodate your preferred appointment date and time; however, appointment slots may be limited.
In order to process your application, all the following documents must be uploaded to this form. You will also be required to bring the originals of these documents to your appointment. You will not be scheduled for an appointment until all required documentation has been provided.

Screening Questions
You are required to answer the following screening questions at the time of submitting your request to book an appointment. A staff person will repeat the screening questions with you the day of your appointment to ensure nothing has changed.
Are you currently in self-isolation or experiencing any of the following symptoms within the last 14 days?
Fever, cough, shortness of breath, muscle aches, fatigue, headaches, sore throat, runny nose.
Has someone you are in close contact with tested positive for COVID-19, is in self-isolation or experiencing any of the following symptoms in the last 14 days?
Have you travelled outside of Canada or been in contact with anyone that has travelled outside Canada in the last 14 days?

Note: If any information is misrepresented or you are missing any of the required documents when you arrive to your appointment, we will not be able to proceed with processing your application and you will need to re-submit in order for a new appointment to be scheduled.
The personal information contained on this form is collected under the authority of the Marriage Act, R.S.O. 1990, c. M. 3 and will be used by Clerk’s office staff to determine whether to issue the marriage licence. Questions about this collection should be directed to the City Clerk at 519-271-0250 Ext 5329.
